Diagnosis and communication skills of cat ringworm

01【Anatomy】

Cat's hair: consists of hair and hair follicles coated at the root of the hair. The hair itself is composed of three layers of internal hair medulla, hair cortex, and hair cuticle, among which the hair medulla is also called the hair shaft.

02【Physiological Function】

1. Protect the skin from physical and chemical stimulation;

2. Regulate body temperature;

3. Sensory organs.

03【Disease Definition】

Diseases caused by keratinous fungi infection of hair shafts and stratum corneum.

04 [Patrogenesis]

Kittens, immune insufficiency cats, and long-haired cats are prone to this disease.

05 [Clinical Characteristics]

1. Skin lesions can be localized, multifocal round or irregular, or systemic hair loss, accompanied by varying degrees of dandruff. The remaining hairs are in a stubble or broken state.

2. Other symptoms include: erythema, papules, scabs, seborrheicus, cervicitis, cervicitis, and cervicitis. Some long-haired cats have subclinical status of asymptomatic cartilage

06【Diagnosis】

1. Microscopy: Stain the lesion at the scraper and a large number of microspores are seen under the microscope.

2. Wood's lamp examination: Some hairs infected with Microsporidium canis will have yellow-green fluorescence, but this is just a simple screening test. False negative and false positive results often occur, with a detection rate of 50%.

3. Hair stem morphology examination: Hair or dandruff is collected at the junction of the lesions, and samples are prepared in potassium hydroxide. Finding hair stems infiltrated by mycelium and conidia can confirm ringworm.

4. Fungal culture: collect hair at the junction of the lesions and implant it in fungal culture medium. A positive reaction (the medium turns red and the colony is white floc) can confirm the diagnosis of ringworm.

07【Treatment】

1. Choose the appropriate drug according to the situation

1) For systemic lesions: oral medicine + shampoo medicine bath; shaved first for local lesions: oral medicine + topical medicine or oral medicine + shampoo medicine bath + topical medicine. Topical medications are available: 1% clotrimazole ointment, 2% miconazole cream. (Topical medication is generally not recommended)

2) Oral medication: Itraconazole 5~10mg/kg (drugs approved for use in cats may cause anorexia, nausea, abdominal pain and increased liver enzymes. It is prohibited for liver disease animals and pregnant animals), PO, SID or terbinepin 30~40mg/kg, oral (side effects may cause anorexia, increased liver enzymes, and gastrointestinal disorders), SID.

3) Shampoo Bath: It may be helpful to take a bath with shampoo (Avibo/Pet) containing chlorhexidine and miconazole before using antifungal agents. Antifungal infiltration agent: 0.2% enconazole solution or 2%~4% lithosulfur mixture.

4) Improve body immunity and skin resistance, assisted nutrition treatment, add unsaturated fatty acids, etc.

2. Treatment needs to be terminated after the subsequent fungal culture results are negative and last for another 3 to 4 weeks before the treatment can be terminated and the examination is repeated every 2 weeks.

3. All carriers who have been exposed but have no symptoms or have symptoms should undergo preventive treatment. That is, topical antifungal lotion.

08 [Prognosis and Communication]

1. Can infect people, isolate and pay attention to personal hygiene, especially children, the elderly and immunosuppressed people.

2. Environmental control: Environmental disinfection can be diluted with household bleach 1:10. Indoor carpets are vacuumed using vacuum cleaners.

3. Prevention and treatment of other asymptomatic carriers or infected people.

4. The treatment of animals with potential immunosuppressive diseases also has a poor prognosis.

5. Long-term chronic infection may contaminate the home and the environment needs to be actively cleaned and disinfected.

6. If you cannot take medicine every day, you can use pulse therapy, but you need to stop the medicine after two negative fungal cultures, with a interval of 3 to 4 weeks.

7. The average course of treatment is 2 months.

8. Wear an Elizabeth collar to prevent the spread of bacteria to the whole body by licking.
